How to Make a Referral

Any individual wishing to use the service or wanting another individual to use the service can make a referral, anyone is able to use our service. To make a referral all you need to do is contact us using the details provided.

The care manager will then arrange to go through an initial assessment of need over the telephone at a mutually agreeable time. At this point we can give an idea of the level of support the individual needs, being able to draw up a plan of care for the client.

The plan of care for a client can often include; Bathing, dressing, medication, feeding, continence care, exercise/rehabilitation, etc.
